Transforming Community Services – Children

NHS Swindon (Swindon Primary Care Trust) and Swindon Borough Council Children Services are working together to meet the requirements of Transforming Community Services where the Primary Care Trust has to transfer the provision of services to a new provider.

Currently PCT children’s health staff are seconded to the Local Authority under a National Health Services 2006 Section 75 Partnership Agreement for the provision of integrated services and the provision of integrated services for disabled children and young people.

These arrangements are monitored by the Children’s Joint Commissioning Board, which is a joint Board of NHS Swindon and Swindon Borough Council.

A Joint Project Board and project team has been established to manage the Transforming Community Services requirements.
